  1. Some exclusions apply. Please visit usbank.com/leveragecategories to see the full list of eligible merchant categories. Please see Cardmember Agreement that comes with your Card for more details. The Rewards Program is subject to change. Rewards are earned on eligible net purchases. Net purchases are purchases minus credits or returns. Not all transactions are eligible to earn rewards, such as advances, balance transfers and convenience checks. "Cash Back Rewards" are the rewards you earn under the program. Cash Back Rewards are tracked as Points and each point earned is equal to $0.01 when redeemed. You may simply see "Cash Back" or "Back" in marketing materials when referring to the rewards you earn. You may not redeem points, and you will immediately lose all of your points, if your account is closed to future transactions (including, but not limited to, due to program misuse, failure to pay, bankruptcy, or death). Points may be redeemed for cash back in the form of a statement credit or deposit to a U.S. Bank checking or savings account only. Points expire five years from the end of the quarter in which they are earned.

  2. U.S. Bank Leverage Visa Signature cardmembers will earn four (4) additional Points for every $1 in eligible Net Purchases spent on prepaid car and hotel reservations in the travel center using your U.S. Bank Leverage Card. Prepaid car and hotel reservations in the travel center are not classified as travel category merchant transactions and are not eligible to receive the additional points for that category. Please allow 1-2 statement billing cycles for your bonus Points to appear on your credit card statement. Refer to the Rewards Center for full details.

  3. One-time 75,000 bonus points (worth $750 cash back) will be awarded if you are approved for a new Leverage Business Rewards Card and eligible Net Purchases totaling $7,500 or more are made to the Account Owner's card within 120 days from account opening. The Account Owner's card is the card assigned to the initial applicant of the account and would not include cards used by authorized employees. Net Purchases are purchases minus credits and returns. Rewards are earned based on eligible Net Purchases. Not all transactions are eligible to earn rewards, such as advances, balance transfers, and convenience checks; upon approval, see your Cardmember Agreement for details. Please allow 6-8 weeks for your bonus points to be credited to your account. Existing and previous Business Leverage accounts do not qualify for bonus points for a similar account type. One-time bonus points do not apply toward account upgrades. Each point is worth $0.01 when redeemed. Points may be redeemed for cash back in the form of statement credit or deposit to a U.S. Bank checking or savings account only. Points expire five years from the end of the quarter in which they are earned. Use of the Leverage Business Rewards card is subject to terms and conditions of the Leverage Business Rewards Cardmember Agreement, which may be amended from time to time. This offer may not be combined with any other bonus offer. Subject to credit approval.

  4. From time to time we may offer you the benefit of our U.S. Bank ExtendPay® Plans, which allow you to pay off balances in fixed monthly payments over time and still avoid paying interest charges on new Purchases. Only your company’s Authorized Officer (AO) may enroll in a U.S. Bank ExtendPay® Plan. All references to “you” and “your” apply to the AO. If your company’s card program is set up with central billing, the AO may select ExtendPay eligible Purchases when viewing the central billing account (which includes Purchases made on employee cards). If your company is set up with individual billing, the AO may only select Purchases made on their own card account. You may designate up to 50% of your credit card line ($100 minimum) in eligible credit card purchases and pay in monthly installments with just a small fixed monthly fee. Only Purchase balances are eligible for U.S. Bank ExtendPay® Plans. Transactions identified as Advances or Balance Transfers (as defined in your Cardmember Agreement), and any interest or fees, including Annual Fee, do not apply. The only Purchases that will appear as "Eligible Purchases" in the enrollment process are Purchases that were made within 60 days prior to signing up for an ExtendPay Plan, are over $100, and are less than your Purchase balance when you sign up for an ExtendPay Plan. Any unpaid balance remaining on an ExtendPay Plan after the chosen pay-back period will be subject to the APR and minimum payment calculation for purchases outlined in the Cardmember Agreement. The monthly fixed fee will be determined when your U.S. Bank ExtendPay® Plan is created and is provided to you before you complete setting up an ExtendPay Plan. This fee will remain the same until the ExtendPay Plan is paid in full. The monthly fixed fee of the ExtendPay Plan is based upon the original principal amount, Purchase APR, and other factors.
